In this webinar we will be hearing from two experts in behavioral
and population health in the primary care setting. We will discuss
the burden of mental and behavioral health on overall wellness
and chronic disease, seeing how appropriate diagnosis, treatment,
and support for these conditions can lead to better health outcomes.
Best practices will be shared to help you and your organizations
understand and mitigate these issues. You are invited to join us in
this journey to expand holistic approaches that can have real impact
for our patients.

    impact on physical health burden, especially cardiometabolic health
  • Discuss the rationale, evidence, and outcomes for integrated care to
    address unmet mental and physical health needs, especially cardiometabolic health
  • Gain familiarity with the practical, clinical tools to use in an existing practice
