Better Health Pathways HUB Receives Ohio Commission on Minority Health Grant!

Better Health Partnership is proud to announce the receipt of a $290,000 grant from The Ohio Commission of Minority Health (OCMH) to help expand and grow the Better Health Pathways HUB, which serves Cuyahoga County’s most vulnerable residents.

The Better Health Pathways HUB is an evidence-based care coordination model that leverages community health workers to help address the health inequities affecting at-risk pregnant women, particularly minority community members who have experienced the greatest rates of infant mortality and maternal morbidity. Numerous people in our community are dealing with serious medical, financial, and social needs, as well as mental illness, and the consequences of structural racism amid the continued COVID-19 pandemic.

The Pathways HUB enrolled 57 pregnant clients in the last quarter of 2022 – the highest number enrolled in one quarter to date. In total, 185 pregnant clients were enrolled in 2022, equivalent to 95% of the goal set by the Ohio Commission on Minority Health and 90% of newborns from HUB clients served had a healthy birth weight.

The goal for 2023 is to serve 190 pregnant people by connecting them to early prenatal care, social services, and other resources connected to the social determinants of health that adversely impact maternal health and birth outcomes. Our goal is aligned with the Ohio Commission on Minority Health to improve the maternal health and birth outcomes throughout the State of Ohio.
