Care Alliance Rejoins the Better Health Pathways HUB

Better Health Partnership is thrilled to share Care Alliance Health Center has returned to actively participating in the Better Health Pathways HUB care coordination network. As a Federally Qualified Health Center (FQHC) with deep roots in Cleveland’s neighborhoods, Care Alliance brings valuable experience in community‑based care and a strong commitment to advancing health equity.

Rejoining the HUB strengthens our shared mission to address social drivers of health through coordinated, person‑centered support. By integrating Care Alliance’s community health workers (CHWs) into the HUB model, we are expanding our collective ability to identify barriers, connect individuals to resources, and improve health outcomes across Cuyahoga County.

“Care Alliance is excited to join the HUB because it marks the progression of what we are working to accomplish as allies in care within the community,” said Jonathan Ballom, Outreach and Engagement Manager. “Our goal in participating in the HUB is to better serve our patients and community members by not only providing quality healthcare services but also strengthening access to resources and addressing social drivers of health in a more coordinated way.

“Our community health workers have been working diligently in the community providing resources, education, and support when needed. Joining the HUB allows us to not only continue that work but expand and deepen our impact.”
