Partnership for Good Health (PGH), a care coordination agency of the Better Health Pathways HUB, recently shared their gratitude for the support of the Three Arches Foundation, whose grant funding has been instrumental in stabilizing their Community Health Worker Program.
"Their investment strengthened the program and expanded our ability to serve community members facing language barriers by improving access to healthcare, health education, and other resources,” said Sara Doran, Executive Director, Partnership for Good Health.
“Three Arches also connected us to the Better Health Pathways HUB, which helps sustain this work by reimbursing PGH for community health worker activities. This reimbursement model, combined with ongoing support, shared learning, and strong community partnerships through the HUB, allows us to continue building on the progress made through the Three Arches grant.”
