About the HUB

Since 2020, the Better Health Pathways HUB has connected thousands of Cuyahoga County residents to care and resources that help them overcome health and social barriers such as insurance eligibility, medical needs, food insecurity, transportation, housing stability, employment, and more. Our Pathways HUB receives referrals from managed care organizations (MCOs) and community-based organizations (CBO) to enroll community residents. 

The Pathways HUB is a care coordination model that connects individuals with complex health and social needs to community-based services through trusted community health workers (CHWs). Each individual is guided along proven ‘pathways’—like housing, food access, or medical care—until their needs are fully addressed. It’s a results-driven system that improves health, reduces disparities, and ensures accountability by paying for outcomes, not just services. It’s a smart, community-centered way to close gaps and build healthier populations.

The Better Health HUB is formally certified by the Pathways Community HUB Institute® (PCHI®)the nation's only certifying body for community-based care coordination for Pathways Community HUBs. PCHI® has built its Model based on a more than 30-year journey that started in Mansfield, Ohio and which has spread across the nation. Organizations seeking certification go through a rigorous peer review process to demonstrate their commitment to offering programs and services that are measurable, accountable, and of the highest quality.
