We accept referrals from many sources, including managed care organizations (MCOs), community-based organizations, healthcare systems, and community residents.
Referrals are shared with our care coordination agency (CCA) partners who assign a community health worker (CHW) to contact and work with individuals in need of care coordination support. Our CCA partners serve adults, infants and children, and pregnant people with complex health and social needs.
