Thousands of our most vulnerable and high-risk individuals seek care and assistance from numerous medical, behavioral health, education, employment, and social service organizations. The Better Health Pathways HUB, a model that coordinates care across sectors, helps move us toward a greater impact on health and well-being in our region for all populations.
