Red Carpet Care

In 2012- 2015, Better Health Partnership launched the first ‘high-utilizer’ total cost of care project in Northeast Ohio called Red Carpet Care. The program delivered estimated savings of $317,000 over one year of continuous enrollment for the 67 participating enrollees of Medical Mutual of Ohio. In addition, Red Carpet Care enrollees generally did better on quality measures for diabetes and high blood pressure than a group of similar patients who were not in the program. Compelling evidence of success also was clear in the stories that Red Carpet Care participants recently shared about the program’s impact on their lives.

CMS State Innovation Model Initiatives

In 2015-2017, Better Health Partnership assisted the Governor’s Office of Health Administration in designing delivery and payment models aligned with the CMS State Innovation Model initiatives and engaged hundreds of primary care providers throughout northeast ohio - helping them transform to the new ways of delivery care under value-based payment contracts.

Pathways Community HUB

In 2020, Better Health Partnership launched the Pathways Community HUB - serving at-risk individuals living in Cuyahoga County. Community Health Workers engage with primary care providers to help their patients get connected to care and resources to meet challenging health and social needs. This is a growing area of collaboration in health care delivery as providers recognize they need to go beyond their clinical walls to influence more than 20% of health that medical care alone provides.
