The Centers Celebrates 5 Years in the HUB

The Centers began working with the Better Health Pathways HUB during COVID-19, a time of significant change and confusion. “We entered into the HUB not knowing how it was going to work and the impact it would have on The Centers or the community. Five years later, The Centers has employed four community health workers (CHWs) and has worked with over 200 members of the community,” said Colleen Ptak, LISW-S, Senior Director Population Health, The Centers.

Initial work focused on educating CHWs about COVID-19 and how to keep themselves and their communities healthy. Work has since broadened and CHWs have helped secure housing for pregnant people while also connecting new and expectant parents to supports that ensure they are prepared when the baby arrives. In addition, CHWs have linked senior citizens to social activities that help reduce loneliness and depression, while improving their quality of life. CHWs have also connected clients to primary care and specialty mental and physical health services.

“The HUB has allowed the Centers to expand its ability to impact the social drivers of health from initially focusing solely on the behavioral health population to now serving more members of the community. The structure of the HUB and the ability to measure client outcomes have been beneficial to the agency and staff. Closing a pathway allows for a concrete representation of success.

Better Health Partnership does an excellent job leading the HUB and provides guidance that ensures staff feel supported and that they are successful. BHP staff have helped to diversify the populations served by The Centers and have assisted with quality improvement projects to meet community needs. The Centers looks forward to continuing to grow in the HUB and have a positive impact on the community,” said Ms. Ptak.
