Care Coordination Agencies

Working Together​

Care Coordination Agencies (CCA) join the Better Health Pathways HUB to access a common system that refers and connects clients with community resources, social services, and healthcare providers. As a contracted partner, CCAs agree to use the PHCI® Model to collect data in a standardized way – including data collection tools, standard pathways to track risk factors, and standard reports. CCAs assign Community Health Workers (CHW) to serve clients and enroll them as members of the Better Health PCH.
