Community Care Agencies

Community Health Workers (CHWs) are trained and employed by Community Care Coordination Agencies (CCAs) to help individuals meet challenging health, social and economic needs. CHWs often live in the neighborhoods they serve and are trusted advocates that can help people take charge of their health and life.
