Our Pathways HUB enables financial sustainability for our partners. From our inception in 2020 through 2025, $2.4M in payments have been directed to our contracted care coordination agencies (CCAs) to strengthen the frontline workforce and sustain evidence-based care coordination. Managed care organization payments, and other funding from grants delivered to agency partners, help CCAs grow their capacity to serve their clients and create a sustainable revenue source.
The below graph shows revenue returned to CCAs (inclusive of Medicaid, Medicare, and grant payments) since the HUB launched in 2020.
