Managed care organizations (MCOs) contract with the HUB to connect their members to care and resources for needs related to the social drivers of health. The HUB partners with the MCOs to pay the HUB's contracted care coordination agencies for services (also known as pathways) delivered and successfully completed.
While continuing to grow and improve service delivery to individuals receiving Medicaid benefits, the HUB model is expanding to serve Medicare members to further align health care and community partners so more people can thrive.
