
Pathways are a foundational tool in the Pathways Community HUB Institute® Model (PCHI® Model) and are designed to support CHW's care coordination work.
CHWs identify health and social concerns (“modifiable risk factors”) that can be changed through specific education and care coordination strategies. Risk factors are translated by CHWs into Pathways to achieve measurable outcomes beneficial to the individual.
