Care coordination agencies (CCAs) that contract with the HUB employ community health workers (CHWs) and their supervisors. CCAs that participate in the HUB have access to the following benefits:

Application of nationally recognized standards and best practices
Access to CHW development opportunities
Opportunity to grow through referrals
Awareness of opportunities to participate in community events
