Better Health Partnership is excited to partner with United Way of Greater Cleveland and Burton, Bell, Carr Development, Inc. (BBC) to support a project focused on improving the health and lives of residents living in Ohio Health Improvement Zones. Neighborhoods that are contained within Ohio Health Improvement Zones have been identified as ranking high in terms of social vulnerability due to socioeconomic and demographic factors that affect the resilience of individuals and communities.
Specifically, this project includes a community-led assessment and engagement project in the east side Cleveland neighborhoods of Central, Kinsman and Buckeye led by BBC in partnership with the Old Brooklyn Community Development Corporation and the Mary Ann Swetland Center for Environmental Health. BHP will be working with a range of partners to help ensure alignment among neighborhood-level CHNAs and the Cuyahoga County CHNA.
As part of this project, United Way of Greater Cleveland will convene public and nonprofit service providers in the three target neighborhoods to identify pain points in the referral and resolution of residents’ health-related social needs and support quality improvement to address residents’ needs. BHP will convene health system partners for quality improvement opportunities regarding screening patients for social needs and linking them to relevant community services.
